UPHP Update: Social Determinants of Health And Connected Communities For Health

Marquette, MI – January 5, 2020 – Katie Poston, Community Health Worker at Upper Peninsula Health Plan, joined Mark & Walt in the Morning to discuss social determinants of health and the UPHP Connected Communities For Health program.

Poston explained what the social determinants of health are and what they mean to the general health of people in the U.P. and beyond.

She discussed the UPHP program Connected Communities for Health and how it addresses the issues related to the various social determinants.

Poston said UPHP members can call them at (906) 225-5964, and their Community Health Workers will work to get you connected with the needed resources.
